Adding date content controls to forms
Explanation
If you want users to enter a date in your form, you can add the Date Picker content
control. When a user selects this control, a drop-down arrow appears. Clicking the
arrow displays an interactive calendar, which can be used to navigate to the desired
date.
To add a date content control to a form:
1 Place the insertion point where you want to add the control.
2 Click the Developer tab.
3 In the Controls group, click the Date Picker control button.
You can modify the properties of this control by selecting it in the document and
clicking the Properties button in the Controls group.

the form.
3 In the Controls group, click
Properties
To open the Content Control Properties dialog
box. The options in this dialog box change
depending on the content control selected.
4 Under Date Picker Properties,
verify that the M/d/yyyy
format is selected
To specify this date format for the field.
To close the dialog box, accepting the default
properties. The settings will become apparent
when a respondent fills out the form.
Click OK
5 In the Controls group, click
Design Mode
To exit Design mode.
Update the document
